WILDHORSE GOLF CLUB - TOURNAMENT ENTRY FORM

2011 JUNIOR MEDAL PLAY CHAMPIONSHIP

Tournament Date: Oct. 8th, 2011 (Saturday) – 7:30-8 AM 1^ST TEE TIME

BASIC FORMAT: Individual stroke play - Each player plays his/her own ball; no handicaps; maximum number of strokes per hole will apply.

ELIGIBILITY: the junior must either have a Wildhorse NCGA junior membership or be approved by our Head Professional or other management staff.

THREE FLIGHTS:

Flight 1 – juniors ages 14 to 17, playing from white tees – 18 holes 7:30am

Flight 2 – juniors ages 11 to 13, playing from red tees – 18 holes 9 am start

Flight 3 - juniors ages 9 to 10, playing from red tees – 9 holes only (with adult supervision)

8:am….. 10^th hole start

Tournament Entry Fee: $20 per player
